Hair Color Request For Review

Approval must be received from the Team Member Assistance Committee (TMAC) prior to color changes. I understand that submission of this form is a request for review. If the hair color is changed prior to the TMAC's approval and is not in accordance with the Swinomish Casino & Lodge dress, hygiene and grooming policy, I will be required to adjust the color to fit within the guidelines. I understand that if I am required to make this adjustment, it would be at my own cost.
